If anyone still struggling with this problem, try something else (it helped me solving the same issue):
1. Go to finder
2. Use Go to option, enter /volumes
3. Rename OS X Base System (with red - before name) to something else eg. Other OS X Base System
4. Re format USB, 1 partition OS X...
I've connected DP2DVI with second monitor with patched 0x7 port and still got restart loop. While trying to hot plug the DP then drivers brakes and system restarts immediately (I can hot plug monitor via HDMI though and it works fine). Any ideas... ? Or the only solution in my case is external...

To be honest - I don't know. I did a clean install of El Capitan, then installed (from Multibeast) Realtek 892 (audio), 8111 (ethernet), Mac Pro 3,1 Definition and ofkz Clover. After that I did your guide.
