In my case exactly the same problem occurs as soon as i reconnect the monitor. Maybe you have some special kexts or boot arguments what will do a reconnect of all usb devices on boot?

Is your Titan Ridge connected to the internal TB header on the motherboard? And also did you connect the Titan Ridge USB to the correct internal USB header? In Windows it should work without any issues out of the box. Check also the bios settings of your TB card.

Maybe it is possible to use a second TitanRidge card but because of a missing second TB header on the mainboard you will not be able to adjust brightness, there will be no sound and no cam. I don‘t have a second 5k monitor so i cannot test it.

The problem with the Designare is that there is no Thunderbolt header on the mainboard. That means you cannot connect the Titan Ridge card. Designares internal Thunderbolt controller has only one Displayport-Input. But the LG Ultrafine 5K needs 2 Displayport-Inputs. Otherwise, you get only 4K...

Yes it works. But you need a mainboard with Thunderbolt header/pins like the Z390 Aorus Pro Wifi or other titan ridge compatible mainboards.

Tried with only one port but the screen stays black. Powered on but black. Maybe there is something wrong with the iGPU. During the weekend I will make a new hackintosh, same components but with a Z390 Designare Mainboard. I hope it will work.

This is what happens when I connect the LG to to the Titan Ridge Thunderbolt Card. Titan Ridge is connected to the Radeon VII with 2 x DisplayPort cables. There is only 1 (not 2) LG Ultrafine 5k connected to the Thunderbolt card but macOS detects 2 separate monitors.
