I gave this a shot on 10.12.6 with my PM981 and had no luck either. Getting the same error as on High Sierra. @wdk23411 seem to be experiencing the same thing on newer Samsung NVMe SSDs.
I've tried:
- HFS+J using the No Convert script from Unibeast to NVMe
- APFS formatted from Disk utility to NVMe
- HFS+J formated in disk utility then allow installer to run its APFS
HFS+J works on the HDD, and trying to CCC it over from HDD to NVMe gives me the same errors.

This fixed it for me!
Took the Serial Number that I used and added enough random digits to make it 17, ran uuidgen and added that under SMuuid. Rebooted and had to call in to Apple and re-verify it.

I had a similar issue. Boot from OSX on Mac HD did not show up. Ended up being that I forgot to reformat to HFS, thumbdrive was still on FAT. Clover worked and was able to boot my system, but it didn't fix the issue when I tried. Now it's working fine.
