Only USB 3 ports working, USB2 ports just powered.

I installed Mavericks 10.9.4 using the recommended Unibeast/Multibeast method and my laptop is close to the same functionality I managed to get with Mountain Lion.

The big regression is that USB 2 ports are not working, so I only have one available USB port left after plugging in the mouse.

GenericUSBXHCI.kext is installed and doesn't seem to generate any errors in system logs.
USB 2 ports still provide power, but neither input nor storage devices are working.

How should I investigate this further?

Use -gux_defer_usb2 kernel flag.
 
Are you using latest version of GenericUSBXHCI.kext?
Yes, I believe so. It's the version included in MultiBeast 6.4.1.

/System/Library/Extensions/GenericUSBXHCI.kext/Contents/Info.plist says

I remembered one significant detail.
During the first phase of installation USB 2 ports were working fine so I checked /Volumes/USB/Extra/Extensions/ and found these drivers that I did not install.

NullCPUPowerManagement.kext
There's also what seems to be a patched version of IONetworkingFamily.kext. I don't remember any option in MultiBeast related to it.
