Unfortunately i'm still not able to reproduce the issue related to your EFI partitions.
Please test the attached version and let me know.
If the issue is still there please open Terminal enterand post HD_Rep_EFI.plist file from your DesktopBash:diskutil list -plist > Desktop/HD_Rep_EFI.plist

After the crash EFI partition was mounted or not?That works indeed as per below screenshot, but when I enter the password to mount the EFI, it crashes. Also when re-opening the app, it keeps crashing.
Theres only 3 EFI partitions in your system that's why you see only 3 partitionsI saw 3 disks (I have more).

Do you both, remember which EFI caused the crash at mount?
@Gompy It still crashes if you reopen the app?It crashes after (the Big Sur EFI) is mounted. I see it mounted on my desktop:
