Wi-Fi ac + Bluetooth 4.0 PCI-E Card Recommendation

It looks like the Wi-Fi ac + Bluetooth card recommended in the Buyer's Guide is no longer available.

Fenvi FV-T919 802.11AC A/B/G/N/AC BT 4.0 Desktop Wifi Card

Can anyone recommend a Wi-Fi card that supports AC and has Bluetooth 4.0 also?

I would prefer to have bluetooth be part of the Wi-Fi card, rather than adding (using) a USB bluetooth dongle, but I'm having a hard time finding a PCI-e card that has both wi-fi and bluetooth AND that works on a Hackintosh.
