I/O Error problem

Hi, im hella confused and out of my mind, what should i do? It's boots but it doesn't let me erase my disk saying that:

"Unable To Write To The Last Block Of The Device"​

My hardware:​

GTX 960,​

I5-3470​

ASRock H61M-DGS​

maybe Disk 2 and Disk 3 have some major issues?

both of them?

as a test, remove all internal and external drives and just try booting with your installer and see what happens

if you have the same issue then use another usb installer
 
I already use another usb installer, it doesn't change anything. Why would i remove all internal and external drives if it's boots i just can't erase my disk so it's useless
 
Do you want me to send you my EFI folder?

We used to see this occasionally but not for a long time now.
