Cannot connect 2 4k monitors

Hi,

I have 2 4k monitors (Dell p2415q: Link) that I am trying to get going on my setup. My hardware is:

i5-6500
GA-Z170N-WIFI
Crucial Ballistix 16GB
Samsung EVO 500GB
GTX 970
Phanteks Mini ITX
EVGA 550W
Wireless Card

There is only one slot in the back of the computer for a Display Port, so I couldn't connect that way. I have also tried
  • DP to DVI adapter does not work at all (in any slot, including with other monitor out)
  • HDMI does not work at all (in any slot, including with other monitor out)
  • Daisy-chaining the monitors with DPi and DPo does get a mirrored display to function properly, but the second monitor is not recognized in the Displays settings. There is no "arrangement" tab
Does anyone know how I might be able to get both monitors functioning properly?
 
At resolutions higher than 1K (1080p, 1900x1080) you will probably need an "Active" adapter, or "Active cable", iirc.

Then there's the whole DP 1.1, 1.2, and HDMI 1.4, 2.0, etc. thing.

When I look for Active conversion cables and adapters I look for the package to say "4K" on it...

What are the outputs of your GTX970? I thought the nVidia GTX970 had three DP 1.2 outputs... (along with DL-DVI-I and HDMI 2.0).
