I have the same card. I was able to use Bluetooth in 10.13.4 (SMBIOS iMac 17.1), albeit with a terrible signal. Now that I have upgraded to 10.13.5, my Bluetooth device does not get recognized.

Great! You have some interesting stuff going on with your audio device. Mine works, but all I did was inject 11 under Devices/Audio. I am also surprised that you do not have Devices/USB/FixOwnership enabled. I seem to need that, but I don't need any inject options enabled in Clover for USB...

I have the same board. When I did my USB checks, HS12 seemed to be active. You do not have this in your dsl. I am not sure what it is (probably internal front panel USB2.0).
From my tests:
HS01 : Internal USB 3
HS02 : Internal USB 3
HS03 : Right port (2)
HS04 : Right port (1): indexed...
The uia_exclude boot options seems to be in error. The HS09 is important for USB-C. :

I alright bought it. It is a BCM9435SZ. It works except for bluetooth. The bluetooth appears to be active in the operating system, but the range is absolutely terrible. It only recognizes devices when they are immediately next to the machine, and even then the signal is messed up.
I have the z370n wifi. The wireless card is a module. I swapped mine out for a broadcom chip (BCM94352Z) and it is working. Bluetooth had hiccups but I think it is ok now (a matter of finding the right kernel extensions).
I went for an Intel i3-8100 CPU. Video acceleration is working.

Switching to iMac 17,1 using Clover did the trick. I no longer have graphics problems with Safari.
