It used to work without an SSDT, but now it isn't working with or without. The only way it seems to work right now is with the tbtonpch ssdt + acpi tweaks.
Have tried cold boot several times, and yeah both bios settings are as you say. Weird!

What are you referring to when you say 'standard hot plug ssdt'? I've only tried the tbtOnPch one with limited success. I can't quite judge the tone of this post, but if it's less humour and more annoyance - I'm sorry for wasting your time man!
