With any app; I have also noticed that on Youtube, when the sound drops, the image also stops and then recovers the frames quickly to align with the sounds that starts again ... not sure if I have expressed clearly, but I hope so.

It's a fresh install. I also a windows on the same computer and the audio works fine. Don't think is an hardware issue. As said, it happens only in macOS and only after reboot, and after some hours it stabilises.
