What method (Clover vs. Unibeast) did you use to install OS X? What are your boot flags? What System Profile are you using? VGA hasn't been supported by OS X in quite some time, and may never be functional. We should be able to get HDMI running though.
In Clover Configurator...
1. Go to SMBIOS.
2. Click the magic wand icon on the right of the screen.
3. Choose the MacPro icon.
4. In the top left corner, change to Mac Pro (3,1) - LGA775
5. Click the "shake" buttons next to the "Week of Manufacture" and "Unit Number" fields
6. Save you...
Verify that your system definition is set to Mac Pro 3,1 (MP3,1). I used Clover as my bootloader, so I can't tell you how to make those changes with MultiBeast. Doing so fixed a nearly identical problem for me with a GTX 750 card.

Okay, I got it working! Two displays, both connected via HDMI Cables.
My problem was that my SMBIOS (SysDef I think for the legacy bootloaders) was set to MacPro 6,1. Though Stork's Guide states that the system definition check is no longer done by the Nvidia...

I've read as much as I can on this, but still can't get anywhere. It seems that I'm the only person in the world who is trying to get this card to run with Clover as my bootloader.
