Bluetooth only works after turning it on and off

I just got a BCM94360CD and it was detected correctly. Wifi is working with no modifications. The BT is also connected but to to connect with an Apple KB and Mouse after booting I have to first turn off the bluetooth (through the menubar) then the devices don't ever show up in the Bluetooth pref pane and in the BT menu item list they only show their mac addresses, not their names.

I had some BT kexts installed to work with a dongle but I've removed them and rebuilt kext cache. CAT ran and everything is reported as correct though I don't have any continuity or handoff features working. My MB and CPU details are in my signature. I have smbios set to 17,1 with icloud and imessage working correctly.

What is up with my BT? anyone seen this before? (only pairs after toggling on/off, only shows mac address instead of device name)

How does your USB Configuration look? Probably better to restore native kexts. Attach ioreg. [Guide] How to Make a Copy of IOReg | tonymacx86.com.
 
When I run kextcache -i I get this:

I have ports disabled in Clover: uia_exclude=HS01;HS02;HS04;HS07;HS08;SS07;SS08;

I used a script to generate ssdt.aml for power management
