I had the same issue, its an issue with AppleAlc.kext and using a DP to DVI adaptor. You can actually fix the issue in clover configurator, snippet of the fix is below, credit to dognmonkey. The Haswell HD4400 and HD4600 have kernel panic reboot after sleep/wake when using AppleAlc.kext...
