Same thing. I can only get dual display by using 2 x mDP. When I add HDMI it gets detected but it's black. If I add DVI it's all green. I haven't played with custom FBs since this card should be supported out of the box.
Can anyone confirm please if custom FB can fix display so...

I assumed that IntelGraphicsFixup.kext is a replacement for CoreDisplayFixup.kext. Sorry. I tried again with both of them installed and I didn't have any luck. Please see logs attached. I will attach picture shortly.
That's what I thought but I wanted to be sure. Unfortunately after installing IntelGraphicsFixup it fails to boot.

Shall I use Inject EDID along with Inject Intel ? Re: DVMT - is the change required before each install ? I did the change before I installed Sierra and it all worked. Also would I have 4k display during installation if there were DVMT-prealloc problems ?
Still same thing. For testing purposes I removed Wifi/Sound kexts to see if that changes anything but unfortunately I am still not able to inject Intel. I am attaching diagnostic files.
