pertinent configs that got me further along with USB3 ports enabled on GA-X99-UD4 under Mojave 10.14.4.
it seems all my usb3 ports are working and showing 5Gb/sec, in addition to all usb2 ports at 480 Mb/sec.

i too saw less than ideal behavior on hot un/plug with 14.3... i'm just getting in the game so i don't have experience with prior on this hardware... it seemed like it might recover sometimes if i waited long enough but i haven't proven that out more than one fuzzy observation

i was previously getting to black screen after the big flip to graphics mode after verbose mode, but i could hear the volume controls from the media keys so i knew it was not crashed and something to work...
Clover/kexts/Other/IntelGraphicsFixup.kext is the one graphics related kext in my EFI bundle that should be considered for /L/E install in addition to Clover... pretty sure it depends on Lilu, so that as well... WhateverGreen seems to be the current go-to alternative aggregating all these...
if you're fortunate enough to have another running system, consider getting the mac system partition to a working state that way, clone it and then try to bang the right things into place from there.
