I've the same wifi card in macOS Sierra and its working fine.

problem reporting files are incomplete
there is no CLOVER folder
restart and generate problem reporting files again
and make sure there is CLOVER folder inside zip file

all kexts should be installed in system
delete AirPortAtheros40.kext
delete/disable IOBluetoothFamily patch from config.plist/Kernel and Kext Patches/KextsToPatch/
\System\Library\Extensions\IONVMeFamily.kext should be removed in post installation from config.plist/Kernel and Kext Patches/ForceKextsToLoad/

I think you should start from scratch
clean install macOS by following this guide
[Guide] Booting the OS X installer on LAPTOPS with Clover

and do attach problem reporting files when you've query.
