Post your GTX 750 Ti NVCAP / IOREG

I have a Gigabyte GTX 750, not a 750Ti, but I hope you find this useful anyway.

Model: Gigabyte GTX 750 OC 2GB DDR5 [
GV-N750OC-2GL]

Connectors: 2 HDMI, 1 DVI, 1 DisplayPort (ver. 1.2)

Status:
Working Ports:
With 1080p monitor: HDMI
With 4K monitor: DisplayPort

Non Working Ports:
With 4K monitor: HDMI (Note: I get the BIOS splash screen but nothing after that)

Not Tested Ports:
With 1080p monitor: DVI (didn't think to test before I got rid of old monitor), VGA (I heard OS X never supported VGA), DisplayPort (old monitor didn't have DP)
With 4K monitor: DVI, VGA (monitor doesn't have these ports)

Without HDMI 2.0, HDMI 4K@60 not supported. Nvidia 9xx supports HDMI 2.0.

Ah, I see. But my Nvidia 750 is supposed to be able to do 3840x2160 @ 30Hz over HDMI. Do the Nvidia Web drivers not support that?

Is the data that has been posted so far, useful and relevant?

How much more data (approximately) do you need, to analyse this issue that we are facing with our 750 Ti(s)? What is the target we are looking at?

Biggest issue, no working 750 TI IOReg

Status:
2x 750 working, same NVCAP
0x 750 TI working
1x 750 TI - no displays/no IOReg
1x 750 TI - no displays/NVCAP same as working 750/install DVI2HDMI adapter
2x 750 TI - DVI working/2 framebuffers missing/unigue NVCAPs

Opinion/750 TI/2 problems
1. 2 framebufers present and working/2 framebuffers missing and not working
2. 4 framebuffers present and not working
 
Here's my IOReg file. Let me know if you need more info. (Don't know why my IOReg file is so large.)
