What Azul patches are you using?

Hi Toleda. Ive tried config-hdmi_hd4600-92_v2.plist.zip Which doesn't seem to help. Problem I'm having is 3 monitors work fine from cold boot but shutdown and restart has problems. Machine goes to shutdown or restart monitors goes black but machine powers off and powers back on OS X keeps reporting it as a Kernel panic now could this be a glitch with the motherboard? Im using 2 HDMI Ports with DVI Port. Do you want me to attach the Kernel panic log?

No, Azul connectors did not change in 10.12.
Port 0x6 and Port 0x7 patches missing.

So I've added those patches. Shutdown the machine connected 3rd monitor. But still got the same problem on restart and shutdown machine goes to shutdown all 3 monitors goes black but the machine stays on then it powers off and back on. Here are the files.
