What worked for me with combo update
pkgutil on nvidia drivers as mentioned by @cristianraiber. I change build number to 17C89 then install
change the OS version in NVDAStartupWeb.kext to 17C89
touch extensions
Same issue here it's annoying. At one point for months I was able to boot into windows, restart, then it booted into clover selection screen but have no clue what made it do that.
Sound just like my issue. I have two monitors (Display Port & DVI). Once I unplug the second monitor, it will boot properly. Once I have both plugging in, I can't boot to desktop. After Apple logo the screen is just black.

I have a odd problem. I installed MacOS Sierra from El Capitan latest Clover but now the only way to boot to desktop after Apple logo is to unplug one of my monitors from a fresh reboot.
The second monitor will work if I plug it in after desktop shows

Nvidia drivers also...
You are getting lag because of the boot argument nv_disable=1. If you have the Nvidia web drivers installed remove that bootflag.
The GTX 970 needs the Nvidia web drivers to work properly.
Try the below argument.
<string>nvda_drv=1 kext-dev-mode=1</string>

By any chance are you using a VGA monitor in your setup? I have a GTX 970 with two monitors (DVI and DVI to VGA) and experience similar problems. I'm running Clover on my setup.
In my case, unplugging the VGA monitor will boot to OS X fine without endless restarts...
This did the trick and it works perfect. Thank you very much for the help.
The fix was going to the BIOS setup:
1. Select BIOS Features.
2. Select Windows 8 Features and pick Windows 8 from the list.
3. Right below Windows 8 Features select CSM Support and set to never.
4. Select Secure...
