Wi-Fi and Bluetooth card for Hackintosh

i'm trying to find a compatible card for my hp probook 4540s. I use a dual band router at home and i want the card to take advantage of the 5ghz band. Also i want bluetooth 4.0 on the same card so i don't have to have a usb adapter. In the future i would like to use the hand off feature in os x Yosemite.

I don't use bluetooth. There are lots of options in the links already provided.
