Volumes "OS X Install ESD" & "OS X Base System" mount when creating USB

Check out the list of mounted images in Disk Utility. Unmount OS X Install ESD image if you see it in the list. It could be there from previous launch of Unibeast.

Reformat disk no help, my front panel USB won't work " OS X Install ESD" but I changed to rear panel USB then works.
