USB formatting issue macOS Sierra 10.12.2

Does anybody else have this kind of issue regarding the usual process of formatting USB pen drives from Disk Utility on 10.12.2?
This is happening in any usb port, with any kind of usb pen drive, using any type of format.

My specs
ASRock Fatal1ty Z170 Gaming-ITX/ac
Intel i7 6700
32 GB ram
Clover Uefi install (EFI folder attached down here)
USB SSDT.aml generated with cnrd script

I was just having this problem! I just erase the USB again, after it's failed. Works!

I tried formatting twice in a row: generally the 2nd time it works!

Unfortunately it doesn't on my system

Had the same issue on my MacBook pro Sierra, It is a new Scandisk 16gig Ulta USB 3.0. pre formatted with Boot partition and FAT. I tried many time.
In the end I took it over to my OLD 2009 iMac running El Capitan and it erased /formatted first time.
Don't know its just an issue with the Sierra or MacBook Pro or Pre format or all of the above. Hope this helps
