Unable to wake from sleep with HDMI

I am unable to properly wake from sleep when my display is plugged in via HDMI.

There are two situations that will occur when I leave the computer unattended for long periods of time:

1. I will come back to my computer to find it's asleep and waking it will lead to 'no signal' OR
2. I will come back to my computer to find that it's still on (fans spinning) but mouse or keystroke input will lead to 'no signal'

I should note that in my Energy Saving preferences, sleep is set to 'never'. When my monitor is attached via DisplayPort, I don't seem to be having these issues. I am not sure if this is caused by a power management issue, a graphic driver issue, both, or neither. I have attempted a few power management solutions, but it doesn't seem to solve the problem.
