[Solved] Remove "empty" Clover boot entry

Hi.
In the attached pictures you can see my current non-hidden boot entries. Thing is: The left and middle entry is exactly what I wanted and need. The Right entry is "empty" and shows no information about the boot location or something like that (see fourth picture). Normally I just would hide the drive "Windows" like I did with many others (Preboot, Recovery, etc.) but than the real Windows entry vanishes as well. And I cannot hide a boot file path because there is none given.
Did anyone of you guys already experience this problem? Is there a possible solution you can think of?

PS: Theme is Yosemite Login btw.

@Aaroknight, please update your profile (personal details) with your Motherboard, CPU and iGPU or Graphics Card(s).
If you have a prebuilt computer enter make and model instead of motherboard.

Did you already disable Legacy entries?
- config.plist/GUI/Scan/Legacy=false

Attach Clover boot.log and config.plist

Profile is updated, thanks for noticing.
Yes, Legacy Scan is disabled.
BTW: When I confirm booting in the empty entry, nothing really happens. Short graphics glitch in clover, then back to normal.

UPDATE:
For whatever reason the entry disappeared and it's exactly how it is supposed to be. I think checking and unchecking the legacy box did the charm. Thanks you very much for your help.
