Severe screen tearing/artifacts. Sierra 10.12.5, Nvidia web drivers enabled but ECC shows no devices

I recently updated from Sierra 10.12.4 to 10.12.5. I had some issues booting so I futzed around a bit then finally found the updated web drivers from nvidia. Now I can boot but have terrible performance and large sections of my screen continue to re-render (shown in the imgur link below). Also note that only one of my two monitors is receiving signal. Not sure what I messed up but I now know to make a backup before updating...

Also shown in the link is my config.plist with NvidiaWeb set to true, my NVIDIA Driver Manager screens with latest version of the web drivers installed and NVIDIA Web Driver enabled (not shown is the ECC tab listing 'no device found' for all four slots), my 'About This Mac' which shows Sierra 10.12.5 installed and system definition as iMac late 2013.

Make sure the Intel HD Graphics 4600 is disabled in your BIOS
Change config.plist/Graphics/Inject/Intel to false
 
Thanks for the suggestions, BreBo. Unfortunately, both of these were already done.

Perhaps it is useful to add that after removing NvidiaGraphicsFixup and Lilu I now get a system panic at about 2/3 through the apple loading screen on startup. I can get into the system again by setting nv_disable=1. Image of -v just before panic below:

I seem to be running out of options but I hesitate to do a full wipe and re-install without first finding out what is causing this. Someone else suggested my system is compatible with a vanilla install (expressing distaste for multibeast) but I'm not quite sure how that process differs from the tonymac installation guide for Sierra. Thoughts?

BreBo, wow, good catch! Sorry! I've uploaded the actual config which I believe is set up as you recommended. This is the config I find in EFI/CLOVER when mounting with EFI Mounter v3 and selecting the drive it suggests.

I'd do a reinstall of macOS. No need to reformat just run the most recent combo update over top of your existing install. Of course a backup is recommended before doing so just in case.

Yes your build is compatible with a vanilla install but it doesn't necessarily differ from using Multibeast. It's just a different post process.
