Random KP/no boot on Lenovo T410

Working on a lenovo T410.
Have mavericks installed and most everything is working.
However, I randomly have instances where after the boot screen, the Apple logo screen comes up and the system loses connection to the HD. It just stops working.
I would say this happens 50% of the time.
If I select "ignore caches" at the boot screen, it will boot every time.
I have tried rebuilding the caches and that really doesn't change anything.

My question is, should I just give up on using kernel caches at boot?
Other than speed, is there any other benefit to using caches?

If there is, where else should I start looking?

I will try to post a screenshot of a verbose boot that fails soon.
 
Have you closed VT-d in BIOS?

A verbose screen shot is pretty much necessary... Or much more information about how you installed and the specifications of the machine.
 
Yes, VT-d is disabled in BIOS.

Machine specs are:
Lenovo T410
core i5 520M
Integrated Intel HD graphics
Intel HD Audio
Broadcom BCM943224HMS (bios patched to remove WIFI whitelist)

Basic Mavericks install, Initially used EasyBeast option w/
PatchedAppleIntelPowerManagement, AppleIntelE10000e, FakeSMC plugins/HW monitor
Patched DSDT using method provided by RehabMan to get battery working
VoodooHDA kext
VoodooPS2 kext
ToledaARPT kext

First:
- make sure VoodooPS2Controller is installed correctly (eg. no AppleACPIPS2Nub.kext or ApplePS2Controller.kext anywhere)
- make sure patched AppleRTC.kext is installed
- install my FakeSMC fork (or a newer kozlek version): https://github.com/RehabMan/OS-X-FakeSMC-kozlek

Next:
- Boot "-f".
- repair permissions in Disk Utility.
- cache in Kext Wizard or DPCIManager.. be sure to monitor system.log for a successful rebuild. If there are errors, fix them.
