Prohibited icon when booting Sierra USB

And then the black and white Prohibited icon.

I've followed the Create Sierra USB thread to the letter. I'm using the imac14-2-uefi-config (using the other uefi config doesn't make a difference).

After a bit more trial and error I'm able to boot to macOS Utilities.

This isn't a terribly helpful answer, but switch USB ports. I was using a USB3 port on the top of my case. Switched to the USB2 port next to it and booted fine. Tried a couple different ports, basically all of the USB3 ports (regardless of where on the computer) would throw up the Prohibited ring.
 
I had to use a rear USB2.0 port (not a front USB 3.0 or 2.0 port). Also, sometimes it just did that, and trying again it would boot.
