One single change fixed the problem for me. I have successful boot on OC 0.7.4 now.

Set this to TRUE:

IgnoreInvalidFlexRatio

• This is needed for Broadwell and older.

No other change is necessary. This advice was NOT needed - in fact it broke my USB mapping.

XhciPortLimit should be disabled

XhciPortLimit needs to be disabled after you have your usbports.kext setup correctly
