No entry sign at boot, waiting on root device Yosemite

Weird issue happened. Usually my hackintosh boots up, freezes most times but have to hit the reset button in order for it to boot successfully. However, last night, I got a no entry sign after apple logo loads. I tried booting in Verbose and I get "waiting on root device". After waiting it keeps appearing and nothing happens. I tried to boot in -v -x -f in safe mode, it boots in safe mode, but the screen glitches in safe mode. Anyone have any idea what the cause is? How do I fix this? Everytime I reset, I get the no entry sign.

Please note that I have been using my hackintosh after installing yosemite from mavericks for several months... this just happened out of the blue!.

Check your bios. Make sure your bios hasn't changed.
 
Check to see if AHCI is enabled. vt-D Disabled. Disable integrated Graphics. Boot with -v GraphicsEnabler=No give that ago.

Waiting for root device is most probably because of below reasons,

1. Disable VT-D/boot with dart=0
2. Connect bootable USB to USB 2.0 ports. Avoid front panel USB ports.
3. Connect HDD/SSD to the first 3Gbps SATA port on your motherboard.
4. Set SATA Mode as AHCI
5. Try another bootable USB


If the above are not resolving your issue, then capture and post a photo of the entire screen after booting with flag dart=0 -v -f if the bootloader is Chimera.
