New partition created after reboot

Upon rebooting, I go into my bios.

In my bios, I go to boot priorities and see that I have many options within boot option number 1, all but 1 with the exact same name: UEFI: Samsung SSD 850 EVO M.2 500GB, Partition 1 (see picture)

Every time I restart my computer, it seems like for some reason a new Partition 1 is created. FullSizeRender-1.jpg

All of the "Partition 1'" options work, as in I boot up and all of my settings and folders are there... just I'm not wanting to have this problem of a new partition being created.

So I then save and exit, with my boot option priorities set as:

Boot option #1: [PO: samsung SSD 850 EVO m.2 50]
Boot option #2: [UEFI: samsung ssd 850 EVO m.2, partition 1]

i then get the following screen. (the P4: WDC WD.... is my 4TB HD)


How do i get rid of this situation?

It doesn't seem to be doing anything bad, but I'm sure that after X amount of restarts, something will happen and it will be bad
