Upgraded to 10.6.3 and it killed audio. Put the 10.6.2 appleHDA kext back in and it works again. Front audio works and so does my digital output (Dolby Digital through Plex) with the attached kext. This is one of the old legacy kexts.

It will work with 10.6.2 AppleHDA with all ins/outs

We'll come up with a solution for 10.6.3 AppleHDA soon I hope.

Does anyone know if the use of a software tool such as "line in" is required in order to pass audio inputs back through the outputs live?

I'm not 100% sure, but I'm going to say yes. I've never been able to do that without some kind of software (never used Line In; normally I use Sound Studio for quick recording or to just pass the input to output).
