Clean install 10.12.5 from USB made with UniBeast.
MultiBeast 9.1.0 ( settings in image ).

Nothing more or less. Setup:
Asus Z87-Plus, i5-4670k, Samsung 850 Evo, 8Gb DDR3

Problem in second picture, also maximium resolution 1920x1080 with HD4600. Any help? :/

For some reason when I install a newer version of Clover it deletes drivers64UEFI/HFSPlus.efi. Without this file I get the boot halt screen. I just copy this file back manually and then it will boot again fine.
HFSPlus-64.efi is a driver extracted from Apple firmware. VBoxHFS-64.efi is an open source driver.
The open source driver is selected by default in Clover.

How do you just boot with that? Where do I find the ig-platform-id? In the config? How do I modify it if the system can't boot?
At Clover Boot Screen select Options > Graphics Injector > *-platform-id

Thanks to RehabMan

Delete or disable the VoodooHDA.kext.
Press Delete Key Upon Booting Up. BIOS > Sound Driver > Disable
After Successfully Booting into macOS then make sure to delete VoodooHDA.kext before enabling the sound driver.
To enable sound again see Post https://www.tonymacx86.com/threads/sierra-desktop-realtek-applehda-audio.197923/
Kernal Panic is caused by VoodooHDA.kext

Moiz,

Your panic is in VoodooHDA.kext. Remove it.

Update 12.12.4 to 12.12.5 no longer boots from NVMe WD Black 256MB. Finally got rebuilt USB installed working. Attached photo shows -v using USB installer. Current time 10:49 MT, I'm not sure why the screen shows 03:33 as this was booted within past 10 minutes. I am not able to get the PC files so will try to attach USB installer version.

I updated to 10.12.5 yesterday, 5/21, and after a couple of reboots, my fans are running at full speed now.
Does anyone know what's up with that?
The fans don't kick in at full speed until after I log in to OS X. They ramp up and down as normal while booting and idles down when it gets to the login screen. But after I log in, they are running at full blast. No background applications running now that weren't running before the update.
I have a Corsair H100i V2 water cooled system with the pump set to maximum rpm via the bios and the fans plugged into the CPU-OPT fan controller for system fan control.

Activity monitor shows only minimum CPU % being used.
And HWMonitor shows my pump running at 2300 rpm, (Motherboard fan 1) and my radiator fans running at 2000 rpm, (Motherboard fan 2)
Normally at idle, my radiator fans should be at 800-1000 rpm with an average CPU temp of 31c.
