I need help.

So, when I plug the USB into my build(3570K, UP5 TH, OCZ VERTEX 4) and chose to boot from it, the following thing appeared:

How can I boot into the Chimera Boot Screen?

I'd try it again, often times this resolves the problem. Also try reseting your bios back to default, then just change SATA mode to AHCI.

I have tried it again. It still didn't work. I have also tried booting in the bios default. Still didn't work. Any other suggestions?

Is your USB formatted correctly? Should bet FAT32/16 and MBR not Mac OS journaled and GUID partition.
Also manually select the drive at the MB logo screen.

I formatted my USB to 1 Partiotion, Master Boot Record, and Mac Os Extended (Journaled). Is that correct/
