HD4600 - Intermittent KP when connecting/disconnecting an external DP display

I got Mojave up and running on my Thinkpad T440p, however I get kernel panics once in a while when i connect or disconnect my external display. It doesn't happen every time, and I don't know what exactly causes it. I've already installed FakePCIID for HDMI Audio. This is what my framebuffer patches look like:
