Hackintosh worked perfectly, until this screen happens on RESET

Please help me out, I will attach any and all info required like Hardware build etc, Bios, whatever is needed.

my build was working perfectly until this screen happens every time I RESET,

is it a hardware issue? or did a new update break the os? any and all help many thanks in advance

You are running OS X El Capitan 10.11.6 (15G31) not macOS Sierra.

Disable the Serial Port in your BIOS.

Your screenshot shows a MacPro3,1 system definition.
The MacPro3,1 does not meet the minimum spec required to install Sierra, you should change it to something that does qualify if you wish to run Sierra.
