Not WiFi related.

"Waiting for DSMOS" without a corresponding "DSMOS has arrived" implies that FakeSMC is missing or not loading.

OK I've fixed "Waiting for DSMOS" -> "DSMOS has arrived" (Installed new FakeSMC)
Then, after installing FakePCIID.kext + FakePCIID_Broadcom_WiFi.kext via Kext Utility, my system is not booting anymore and showing the same error message as before.

Remove FakePCIID.kext.

Then provide data...

Comments:
- config.plist/RtVariables/CsrActiveConfig should be 0x67 (you have 0x3).
- config.plist/SystemParameters/InjectKexts should be "Detect". You have true.
- all kexts you need should be installed to the system volume.
- "essential" kexts should be placed in EFI/Clover/kexts/Other
- CPU PM not implemented
- QE/CI not implemented (Nvidia graphics drivers needed, but not installed?)
- USB not implemented correctly (port limit patch is only for short term use)
- fcvo patch is disabled, but required for BCM94352

Suggest you try to correct a few things with your configuration... and make sure you're using the current build of FakePCIID.kext and FakePCIID_Broadcom_WiFi.kext.

Your issue could also be ACPI related (eg. no accommodation for _OSI("Darwin") in your ACPI setup).
 
Thanks RehabMan, I included an ioreg while the problem is occurring, it indeed shows IOBluetoothActive as false. Surprisingly BT parameters say that it's active. Thanks for looking into it.

You will need to provide an ioreg capture in the same situation. An ioreg from the scenario prior to the problem is useful as a comparison, but not useful to see what is happening in the error situation.

This "Sophie's imac_BT down.ioreg" shows bluetooth working, devices connected, etc.
 
Thanks for the thread. I've succeeded with WiFi , but

1. Bluetooth seems to show up everywhere it should, System Report even recognizes nearby devices, but these do not show up in the Bluetooth utility. I get nothing when I cat the log as described here:

so I think BcrmPatchRAM2 must not be loading the firmware?, even though recent versions of that .kext and BcrmFirmwareRepo are installed in L/E/ by kextBeast

2. OS 10.12.2 (16C67) / GA-Z170X-UD3 Ultra / F21 BIOS / i7 6700 4GHz / GTX 750Ti SC
Made my own card using a BCM94352Z NGFF M.2 + adapter as described in TonyMac's guide.

3. I used this guide, Post #1, procedure 1.1 , 2.1-2, and I think I installed the appropriate handoff and 5Ghz patches under 3. Tried and hopefully successfully undid some alternatives.

Attaching requested files, except I don't see an airportd in the Console.

I have the same issue. I have WiFi working fine. Bluetooth seems it's installed, but I can not connect any devices.
 
Hi is there any support for the Atheros AR5BXB72 Bluetooth side of the card ? As the wifi works ok
I have showing instead of the Bluetooth info in network active services
Built in serial port (1). Type PPP (PPPSerial) hardware modem
Bsd device name serial1
