GTX 760 - 2nd DVI Failure

Hi,

I have a older hackintosh system that back in March I upgraded the video to a used GeForce GTX 760 4GB card. I had it working great on the Mac under Sierra and also when I booted into Windows 10. This week though I booted the system one time and my second monitor was acting like it wasn't connected. It is dead in both OSes, and I have tried swapping cables, monitors, etc. so it is really the second DVI output that has died, but yet the first DVI output still works fine! I did get it so that I could play games on the Windows side sometimes, and while playing I know that it would get exercised pretty well, but I don't think that should have contributed it to failing early.

1. Has anyone seen a failure like this? I am wondering if by some remote chance there is something I can do to fix it?

2. Could it be that I just got a card that finally failed and I should just look at trying to find another to replace it, or is there some reason that if I get another this will just happen again?

3. Any other comments or thoughts on what to do would be appreciated!
