El Capitan White Circle of Death

Hello everyone,

I'm currently trying to do a fresh install of El Capitan but every time a boot OS X from my usb, a screen pops up with the apple logo and a progression bar, when the progression bar reaches half way, get the "white circle of death" (circle with a line through it).
I have tried boot flags such as:
npci=0x2000, cpus=1, nv_disable=1

and they all lead to the same result. In addition, i tried placing the usb into a port connected directly to the motherboard, disconnect all other devices connected to the usb port except mouse and keyboard and that didn't work.

I also tried booting in verbose mode with the boot flags i mentioned and got these results:

after that error i also added the boot flag -s and it then said "Boot failed, sleeping for 10 seconds before exiting..."
I used Unibeast 6.2 to create the bootable usb which installed the clover boot manager on it.

My current build is:
Sabertooth x79 M/B
EVGA Nvidia GTX 760
OCZ Vertex SSD (120GB)
intel i7 3820

I spent 3 long days trying to figure out how to get this mess fixed. Any suggestions or ideas is truly appreciated!
