Intel NVMe SSDs are known for compatibility and performance issues with macOS/OS X.
Poor choice for hackintosh.

Probably not related, but MacBookPro11,3 is a poor choice.
Use MacBookPro11,1.

Use the gen_debug.sh tool mentioned in the FAQ, that way it is less likely you'll omit something.

Yeah, as RehabMan said it's just incompatibility between macOS and Intel SSD (maybe some others too). I didn't solved it but made that bug appearing as rare as possible: I disabled pmset standby and "disk sleep" option. It's still may happen but once a day, not 6 times.
