My OSX shows alot of:
AppleUSBHostController::hardwareExceptionThreadCallGated: 0x00000010,

and "attempting recovery.. did not synchronize"

when booting,
then suddenly shows forbidden sign.

I have dual boot Windows and OSX on the same computer,
i have run it like this for over a year, but managed to delete my EFI folder
and had to install both OSX and windows all over.

When booting from installer usb, the installer cant find any disks.
but terminal "diskutil list" suddenly shows 20 disks when i only really have 2 (usb installer and ssd with 2 partitions)

My EFI contains these kexts on USB installer:
kexts.png

My EFI (on mac itself) also contains:
- USBinjectall.kext
- USBXHCI something .kext
- usb ssdt (one i just found online for coffee lake)

I've installed Windows and OSX 5 different times, and it works for like 2-3 boots,
then suddenly wont boot with this error..

Please help! This happened in the middle of exams...
Couldn't be more frustrated.

UPDATE:
Managed to get it to work using custom ssdt for usbinjectall.kext

Did you manage to resolve this? I’ve started to get these errors now and random reboots with no error log. All started when I installed an 970 NVMe
