Bluetooth not available on wake

I was referred here by the Moderator toleda in the Broadcom WiFi/Bluetooth [Guide]. The zip file includes all troubleshooting files requested under that thread.

Bluetooth is usually recognized on boot, the IORegistry is normal and so is bluetooth behavior. However, after closed lid sleep and on battery power (haven't noticed this when plugged in):

  1. the bluetooth shows 'Not available' and
  2. the IORegistry sometimes shows a double connection under USB (but not indicated in attached ioreg file)
  3. System Report/USB shows a double entry for the Bluetooth/Wifi card
  4. IORegistry shows Bluetooth no longer recognized under IOService:/AppleACPIPlatformExpert/... (See Screenshot)
  5. Often a few minutes after waking up with the 'Bluetooth not available', the laptop reboots and has the following error report:
