Gigabyte GeForce GTX 750 Ti (2 x HDMI + 1 x DP + DVI)
nVidia latest Web Drivers 378.05.05.25f01
Dell P2416D 2K monitor

Using Lilu.kext & NvidiaGraphicsFixup.kext

I can only get display working on a middle HDMI, other HDMI & DP produce NO signal, did not try DVI (so no multi-monitor possible)
Any idea how to get all ports to output? I really only need DP to also work...

EDIT
Turned out that I had to set Video in BIOS to be PEG, as Auto was giving the very problem
Once set, DP works (as well as HDMI, DVI untested)

i've same GTX and problem like you. let me try to change to PEG this night. now im not at home
