Atheros Bluetooth: IOath3kfrmwr.kext: Not Working (AR3011)

Hi I need help to fix my AR3011 Bluetoth problem.
SO I thing that problem comping from USB. but I can not fix it.
my machine is Toshiba L755-1nk , also I have UP720200 USB3 / 1 port that I active work with fill speed but in not showing in system report / usb.

I have DSDT patch for UD720200 , and I load in PR02 , but still same problem , no Bluetooth.
please help

No bluetooth on the USB bus.
Test bluetooth on Windows to insure your hardware is functioning as you expect.

I solved 5min ago. I unplug and plug again wireless cars from PCI slot and everything run..
but after sleep bluetooth disable and after restart not comping. I should again unplug/plug wireless to came back bluetooth .
did you know how to fix this problem ?

Did you install IOath3kfrmwr.kext to the system volume?

Hi again.
I have problem with my AR9285 + AR3011 BT.
So here is problem wen I start Laptop Toshiba L755 wi-fi was work great, but not BT device. I unplug wireless card and put it back and bluetooth is loaded and work great, but I should restart machine because Wi-Fi give error. and after this restart everything is OK.
till not should down. If I make only restart there is no problem.
But If I make should down I should make everything again to unplug /plug wireless / restart.
what I should do to fix the problem ?
