Sorry, no Idea I cannot edit then add attachment. here is my EFI without themes directory.

[EDIT]

@vonrussel, have you tried to upgrade to Mojave 10.14.1 yet? I tried following this guide https://www.tonymacx86.com/threads/guide-intel-framebuffer-patching-using-whatevergreen.256490/ and https://www.tonymacx86.com/threads/release-intel-fb-patcher-v1-6-5.254559/ but couldn't get HDMI output at all. I do not know how to patch "connector" properly even with fb-patcher, can anybody help please? I have attached the patcher screenshots here, would appreciate if someone can point me to the direction in knowing what options to pick.

Not yet upgraded to 10.14.1 but I have converted my KextToPatch patches to framebuffer patching. Here is my EFI with Devices -> Properties with patches for connector.

I have tested on 10.14.1 using your framebuffer patching and it works perfectly. Thank you.
May I know which guide you went through to produce the framebuffer patch please? I would like to know how I can learn to patch them systematically.

Good to know I can update, thanks!

I am fairly new to hackintoshing, what I did is converted my KextToPatch patches to framebuffer patching using Intel FB Patcher. You can reorder connectors in it by dragging the row item. Also copied the BUS ID because my patches wont work if I use the default one. You have to figure this one out which port works, in our case, port 6 works for hdmi. I cannot fix the ports for dvi-d yet since I do not have extra monitor (maybe in the future will work on this). Maybe you could check if DVI-D works on our patch, for dual or just using the DVI as main.

Hey! I have two monitors. I just tested the DVI-D output. It works as main only, but I cannot get both HDMI and DVI-D working at the same time. Any idea what I can try so that dual monitors would work? Related to framebuffer too?

Can you check what port is hi-lighted when using DVI-d?

OMG, this worked for me on my gigabyte H370m to solve the lack of HDMI problem!! How did you end up finding the original kexts you converted to WEG? I'd like to make a tutorial so no one has to go through this again.. :)

Interestingly, it still shows it is a DP, instead of HDMI:
