I have the BCM94360CD card installed. Under Sierra 10.12.4, 5Ghz ac networks do not work. Occasionally the 5Ghz wifi network will appear in System Preferences but the mac is unable to connect to it. I cannot seem to connect to the 5Ghz network in Windows either so I don't know if this is a hardware issue. But seeing as my Macbook Pro can connect to the 5Ghz network, this card should work for both OSes. Handoff and bluetooth work out of the box.

However, the card doesn't appear under PCI. Instead, Realtek Ethernet is listed as a PCI card even though it is part of the motherboard. Though I suspect this is not an issue?
