If your 1050Ti card has 1xDVI, 1xDP and 2xHDMI outputs, you can only use one of the two HDMI. Therefore you are limited to max three monitors via DVI, DP and 1xHDMI.
You may not be able to use the 2nd HDMI port directly. You may have to use a DVI-HDMI or DP-HDMI adapter as in my experience the 2nd HDMI port is not supported by OS X. Let us know how you go as my information may be outdated...
BTW you should be able to run the EFI version of memtest86 which will have a much newer version (v7.4). At startup choose the EFI option in the BIOS boot menu.
I normally have my GTX 1050 Ti connected to a BenQ 4K monitor via DP. However I just swapped it to HDMI (using the monitor's HDMI 2.0 port) and it comes up as 60HZ, according to the info screen reported by the monitor itself. I'm also using a BJC HDMI 2.0 cable.

Thanks for the confirmation. Looks like the same issue I'm having. It's not a deal breaker by any means but I'll need to run some long term audio tests (i.e. play music through it for many hours) to ensure that it doesn't cause any lingering side effects.
When your adapter arrives and you plug it in for the first time, can you check your System Preferences->Sound->Output and see if it appears immediately or whether it takes 20-30+seconds before it appears as a sound output device?
