I also tried to get LinkAggregation working on my GA Z77X UD5H and finally gave up with onboard lan.
Up to OS X 10.0.x Apple had some native drivers which worked with a dual nic HP NC 360T. But with the new OSes they lose connection under heavy load.
I tried to use the SmallTree drivers and...
Oh, and where?
In a kext linked to this thread there was a version with two places:

Hello,
since several weeks I try to get FakePCIID working for my dual NIC HP NC360T on OS X 10.11.2 / Clover. The AppleIntel8254XEthernet.kext doesn’t work reliable on my GA-Z68MX-UD2H-B3 - yesterday I installed the new AppleIntelE1000e.kext which seems to work so far with Link Aggregation...
I get:
"Mac OS X version 10.10.3 (14D136) is not supported with this package. Please see NVIDIA’s website for further driver information."
I thought I was on latest Security Update 2015-002 1.0???
Unfortunately I cannot comment on your Samsung monitor, but I have a LG 34um95 with the same resolution and similar ports.
I had problems when I tried to quit the monitor energy save state of the monitor. Those steps fixed my problem:
- use the latest FakeSMC.kext & HWSensors from...
I had the same problem! I use Ozmosis which has an old version of FakeSMC.kext built into the bios. Installing the newest FakeSMC from Multibeast into /S/L/E fixed my fan-problem with the GTX 760. So you might check, if your FakeSMC incl. HW-Sensor-kexts is up to date...
Suddenly I have the same problem with a Z77X-UD5H. I found something here: http://www.tonymacx86.com/mountain-lion-desktop-support/70856-fix-inside-ga-z77x-ud5h-usb3-problem-under-ml.html#post439512, but the firmware update of the via VL810 hub to v9.6 did not help.
Perhaps your Z97 has also...
With the on-board NIC on my GA-Z68MX-UD2H-B3 I often had problems connecting to the LAN (having to boot Windows and then OS X to get it working again).
Thus I bought a HP NC360T PCI-Express Dualport GigaBit Adapter which works OOB for some users.
On Mavericks the builtin driver does not...
I have almost the same board (GA-Z68MX-UD2H-B3 rev. 1.3) and I am able to boot with VT-d option enabled in UEFI and WITHOUT any boot flags. I use VT-d in my VmWare-machines.
I successfully updated my GA-Z68MX-UD2H-B3 (rev 1.3), i5 2500K, 16 GB, OC 4,3 GHz to Yosemite which is quite similar to Storks board. For the first successful boot after the Installation of Yosemite and Multibeast I had to use the boot flag "-f".
I'm using the beta-UEFI-Bios and a DSDT-free...
