What’s weird is I tried installing a new Big Sur install on an identical computer but couldn’t get the metal to work. Tried different versions of Big Sur installer and also EFI folder from the working one.
My test is if the dock is solid then driver didn’t load, supposed to be translucent. And looking at System report - Extensions - AppleIntelHD4000Graphics driver loaded or not.
Also System report - Graphics/Displays, when driver doesn’t load there’s no entry for Metal status.
Let me take back this post. My issue solved by AMT set to enable in BIOS setting.

Used Big Sur installer 11.0, HD4000 works. Even after updating to latest 11.2.3
Fresh install using 11.2.3 installer, HD4000 driver does not load.

This is only for the Dell Optiplex 7010 USFF with i7-3770s which uses the Intel HD4000 GPU. I don't have the other form factors so I won't be able to answer any questions about them.

Just like the OP, no USB 3 shows up in Hackintool or any of the utilities. OP solved his problem by editing the DSDT. OpenCore guide says...
