I fixed this issue. My USB 3.1 controller is not compatible so I disabled the controller and enabled the IOAPIC 24-119 Entries again. Now works with no issues, still getting USB 3.0 so not concerned about the 3.1 controller.
I had to disable it on my ASUS E3 Pro Gaming V5 Mobo to get the USB to work. With it turned on, after Sierra booted, I was unable to use keyboard or mouse. I disabled it and USB then worked.
